Appointment Information
Please note Peachtree Women's Clinic is part of Atlanta Women's Health Group (AWHG).
Appointment reminders will be sent via text, email or voice message 3-5 days prior to your scheduled visit and will include appointment date, time and office location.
A link to our online check in process, Phreesia, will be sent to you via text or email. Please complete check in process 1 day prior to your appointment.
Due to COVID19 Protocol, patients are permitted to bring one person to their appointment with them. All safety precautions will remain in place including mandatory wearing or masks and temperature screening at the door.
Please arrive 10-15 minutes prior to your scheduled appointment to update your information and insurance.

Payment Policies
Self-Pay Patients
Payment will be due at the time of service.
You may call the office to obtain an estimate on the cost of services prior to your appointment.
Lab work, pap smears and other testing in the office is billed by PhyTest, and you may receive a bill from them for additional services. Again, an estimate of the cost of these services can be obtained by calling PhyTest prior to your appointment.
Commercially Insured Patients
Appropriate Co-pays will be due at time of appointment.
While many insurances will cover a Wellness/Annual Exam, please check your insurance to determine if there is an additional co-pay.
For non-Wellness or Problem Visits, there may be a co-pay or cost associated with your visit as it applies to your deductible. Please call your insurance prior to your visit to determine you benefits.
Medicare & Medicare Advantage Plans
Medicare and Medicare advantage plans will not cover a well woman visit. We will have you sign an advanced beneficiary notice (ABN) stating that you are aware that Medicare does not cover the well woman visit. We will bill the insurance, and you will receive a bill for what Medicare does not cover. Sometimes a well woman visit is scheduled but it can be billed as a problem with the Medicare pap (Q0091), breast/pelvic exam (G0101) being added to the problem. Our office will bill Medicare directly for any problem visit, ultrasound, biopsy and mammogram. Medicare pays for most all problems and procedures.
Laboratory
Depending on your insurance carrier, the charge for your laboratory tests will be billed by Quest Diagnostics, LabCorp, Genzyme, NTD Laboratory. Self-Pay patients will be billed through Phytest, our laboratory billing service. Please call the laboratory directly to determine your benefits and coverage.

Administrative Services Fees (ASF)
Our office collects an optional Administrative Services Fee, which covers the completion of certain forms such as Long-Term and Short-Term Disability, Family Medical Leave Act (FMLA) Paperwork and other documents for one full year from the time of payment.
- For GYN Patients: $15 ASF
- For OB Patients: $75 ASF
You are not required to pay the Administrative Services Fee, however, if you require forms filled out by our office due to surgery, medical leave or maternity leave each form will be subject to individual charges based on complexity.
